“Transition” is the name of the process of moving from school-age years to the expectations of a full life as an adult. The purpose of this Transition Directory is to provide an understanding of the process – and the roles and responsibilities of all stakeholders involved in the process – of supporting individuals to be successfully employed after leaving school.
